Masks are required in our office at all times. We sanitize items and spaces between each appointment and frequently throughout the day. Please reschedule your appointment if you or someone in your household is not feeling well, has a fever or cough, has been exposed to or tested positive for COVID-19 or another viral illness within the past month, or has traveled to another state or outside the US within the last month from the date of your in office appointment. If you are concerned, please call us to discuss. We understand cancelling your appointment last minute might be necessary due to illness, therefore the cancellation/no-show fee policy will likely not apply.
We do our diligence to the utmost to ensure all of your information remains confidential. We will not share evaluation results, data, diagnoses, reports, forms, or any other information without your prior written authorization. If you would like for us to speak with another individual, please request a release of information. In addition, as required by law, we are mandatory reporters, and if a child or other invididual reports and/or appears to be in danger to harm self or others, or if the individual has been harmed by another individual, we will notify the appropriate agency.

When scheduling appointments, you will be contacted via phone call and/or email. We require a written confirmation in order to secure your appointment. Without a confirmation, your appointment will be cancelled. In addition, if you need to reschedule, do so at least 24 hours in advance. If you do not inform us of the cancellation in a timely matter or if you miss your appointment, you will be responsible for a $100 no-show fee; this fee is not covered by insurance.
